When I started driving my parents were very insistant that I have roadside assistance. I didn't really understand or care, but they paid for it so why not? More than two-thirds of the 6.5 million emergency room visits for people under age 65 are avoidable, according to a study released by Truven Health Analytics. The study examined insurance claims data for over 6.5 million emergency department visits made during the 2010 calendar year and found that just 29% of patients required immediate attention in the emergency room.

About 72% of emergency room visits made by patients with employer-sponsored insurance coverage are for causes that do not require immediate attention in the emergency room, or are preventable with proper outpatient care: 24% did not require immediate attention; 42% received care that could have safely been provided in a primary care setting; and 6% received care that would have been preventable or avoidable with proper primary care.

The analysis surmises that diverting just 10% of these unnecessary visits to an office setting would result in a net savings of $18.68 in total allowed costs per health plan member, per year. Based on the 24 million enrollees represented in the Truven Health MarketScan databases, this represents a total potential savings of $461 million each year.

Based on clinical records and the patient's primary diagnosis, Truven gave all visits probabilities of falling into four categories:

1. Nonemergent - medical care was not required within 12 hours.

2. Emergent/primary care treatable - medical care was required within 12 hours, but it could have been safely provided in a primary care setting.

3. Emergent (preventable/avoidable) - medical care was required within 12 hours, but could have been dodged with timely and effective outpatient care.

4. Emergent (not preventable or avoidable) - medical care was required within 12 hours at the emergency room.

A diagnosis of "abdominal pain unspecified site" is given a 67% probability of second category and a 33% probability of the fourth. Truven's report says this method was used to acknowledge the "gray areas" of claims and diagnosis.
"That is, it recognizes the fact that, based on the diagnosis information on an ED claim, it is impossible to say with certainty that 'X' visit was not necessary, only that 'Y percent' of the time it has been shown to be unnecessary," the report reads.

Across six broad health plan types, the percentage of nonemergent visits differed only by 2.6% and of Category 4 visits by 2.5%. More substantial differences were seen by gender: Women (who accounted for 17% more ED visits overall), were more likely to make Category 1 visits and less likely to make Category 4 visits than men.

Striking variations were also seen by age - more than 75% of patients age four and under were in the ER for Category 1 or 2 visits. The second largest age group for these most-avoidable ED trips was those in the 20-29 range.

"Given the extremely high cost of ED care versus care in alternative settings, the impact of any program that shifts even a small percentage of care by helping this population establish appropriate care relationships and providing better access to the appropriate treatment setting will pay large dividends."
In 2010, the average ED visit cost $1,316 and the average office visit cost $145, so the net saving of every emergency visit replaced with a trip to primary care would have been $1,171.

A record number of Americans are going to emergency rooms for dental treatment, a problem the dental industry says is straining the nation’s health care system and increasing its costs.

Research out Tuesday from the American Dental Association found the number of dental ER visits in the United States jumped from 1.1 million in 2000 to 2.1 million in 2010. And that comes with a big price tag: Those visits cost the health care system up to $2 billion annually.

Most of those patients are young adults, the analysis found. That’s due to decreases in private dental insurance coverage, combined with significant reductions in adult dental Medicaid programs that have hit young adults particularly hard, the nation’s largest dental association said.

Not only do those ER visits strain the system, they do little to actually fix dental problems. For one thing, few hospitals have dentists on staff able to provide comprehensive dental care. That means that in most cases, staffs provide patients with prescriptions for pain or antibiotics for infection, which ease short-term symptoms without solving the long-term underlying dental problem.

“Without further interventions from policy makers, dental ER visits are likely to increase in the future, putting additional strain on the health care system and increasing overall health care costs,” Thomas Wall, a lead author of the briefs, said in a statement.

The analysis also pointed out that the Patient Protection and Affordable Care Act does little to address the issue of rising numbers of ER dental visits because “it does not mandate dental benefits for adults, nor are dental benefits likely to be included in the essential benefit packages in insurance plans sold through most states' exchanges under the law.”

The ADA has been critical of PPACA for its neglect of adult dental care, calling the law “a missed opportunity” that fails to address critical dental issues.

Experts have warned the law could significantly disrupt the dental benefits market if employers drop coverage for their workers or workers opt out of duplicative plans for themselves to pay for their children’s benefits. PPACA mandates dental insurance for children, as its definition of “essential health benefits” encompasses pediatric services, including oral and vision care.

Meanwhile, the ADA is calling on policymakers to look at other ways to improve access to dental care for adults, including programs aimed at diverting dental patients from ERs to community health centers or private dental practices.

We need another bathroom!!

We need another bathroom! This has been my thought for the past three years. You see about 3 years ago my house burnt down. I have spoke of this in other PR’s. We were living in a rented house while we remodeled my new husband’s one bedroom one bath, bachelor pad! Well, after the fire we were all forced to move into his trailer! Our plan was to turn his single wide nightmare into a double wide.  Now, we didn’t want to take out a loan to do this. My husband is a carpenter, he builds stuff for a living, so he is more than capable of building us a house. So instead of the quick fix, hire someone to do it. I let him build it.

It’s been a slow process. Three years later its nearly done!!! Except only one bathroom still! Three years ago it wasn’t as big of a deal. My girls were 8 and 10. They didn’t require a lot of bathroom time. Now, they are 11 and 13 and getting into the bathroom in the mornings can be challenging.

Now, I know your asking yourself, “What does this have to do with your business Wendy?” Well, I’m so glad you asked. When I started my business I knew it would be a slow go.
